Prior authorization that lives on the order, not in a spreadsheet.
For most DME and HME providers, prior authorization is where an order stalls. The request sits in a payer queue, the follow-up lives in someone's inbox or a spreadsheet, and the authorization number has to be found and keyed onto the claim by hand once it finally comes back. Medicare fee-for-service requires prior authorization for power mobility devices, certain orthoses, lower limb prostheses, pneumatic compression devices, and pressure-reducing support surfaces, and Medicare Advantage, Medicaid, and commercial plans each layer on their own lists and rules.
Curasev builds prior authorization into the sales order instead of treating it as a separate step. When a product's fee schedule requires prior auth, adding it to the order opens an authorization request (PAR) automatically. The request is tracked on the order line, followed up through assigned tasks, and its approved authorization number is applied to the claims for that order without anyone re-keying it.
This page walks through how that works. If you are comparing platforms, ask every vendor, Curasev included, to show the same thing live: a product that needs prior auth added to an order, the request faxed, its status tracked to approval, and the authorization number landing on the claim. For the broader evaluation, see our DME software comparison, and for the policy backdrop, our post on prior authorization reform.
Prior auth rules are set on the fee schedule, so the system knows which products need authorization for which payer. When one of those products is added to an order, Curasev opens a PAR automatically. Nobody has to remember that a power wheelchair base or a support surface needs authorization for this payer. Staff can also open a PAR by hand for any product, which covers payers whose requirements haven't been configured yet or one-off requests from a plan. Because the request starts on the order, it moves with the order through sales order orchestration rather than living in a separate tracker.
Each PAR records the diagnosis codes, the ordering physician, the payer, and the products being requested, so the request and the order stay consistent with each other. Requests can be faxed to the payer directly from the app, without exporting a form and sending it from a separate fax tool. If your referrals arrive as faxes and PDFs in the first place, Seva AI document intake handles getting CMNs, SWOs, and prescriptions into the order before the authorization step begins.
Payers rarely tell you when a request has stalled. Curasev keeps follow-up moving with assigned tasks that carry due dates, so each open request has an owner and a next action. Authorization status shows on every order line, color-coded, with the expiration date on hover. An intake coordinator or biller can see at a glance which lines are approved, which are pending, and which are close to expiring, without opening each request.
An approval is not permanent. Medicare's affirmed decisions are valid for windows that vary by category, from one month for pressure-reducing support surfaces to six months for power mobility devices, and commercial and Medicare Advantage authorizations carry their own expiration dates and unit limits. Rentals and recurring supplies outlast a single approval, and patients change plans mid-service. Curasev tracks renewals, recertifications, and payer switches against the authorization, so a capped rental or a resupply order doesn't keep billing under an authorization that no longer applies.
For Medicare, the Unique Tracking Number from the decision letter has to appear on the claim, and a missing or mismatched authorization number is one of the most avoidable denial reasons in DME billing. Curasev puts the authorization number on the claims automatically, including claims that were created before the approval came in, so nobody has to go back and correct them by hand. From there, automated billing scrubs the claim against payer rules before submission.
